Why this template works

Feedback forms have a hard problem: visitors hate filling them out. The smiley-face quick rating (good/neutral/bad) gets 5-10x higher response rates than text-field forms because the cognitive load is near-zero. This template captures the rating in one tap, then conditionally asks for a follow-up — only "bad" or "neutral" responses get a "what went wrong?"
